Does CCM/RPM cost me anything?
No. Both programs are covered by Medicare Part B. There is no copay, no premium, and no out-of-pocket cost for eligible patients. If you have a Medicare supplement (Medigap) policy, it may cover any applicable coinsurance.
Do I have to change my primary care doctor?
Absolutely not. HEAL Medical Group works alongside your existing primary care provider, specialists, and other providers. We coordinate with them — we do not replace them.
Do I need to come into the office for CCM visits?
No. All CCM care coordination happens by phone, secure message, or video — from the comfort of your home. RPM devices are shipped directly to you.
What devices come with RPM?
Depending on your conditions, you may receive a blood pressure cuff, blood glucose monitor, pulse oximeter, or weight scale — all FDA-cleared and Bluetooth-enabled. Readings transmit automatically to your care team.
How do I cancel if I change my mind?
You can withdraw from the program at any time with no penalty. Simply contact our office and we will process your withdrawal immediately.
Can I enroll even if I already have CCM elsewhere?
Medicare only allows one CCM provider per patient per month. If you are currently enrolled with another provider and wish to switch to HEAL Medical Group, contact us and we will guide you through the transition.
