We operate HEALBRIDGE (CCM/RPM platform), VetBridge (veteran health services), and a network of clinical and population health platforms serving Medicare and Medicaid patients across multiple states.
We built HEAL Connect because we lived the problem firsthand. We know what it means to need a collaborating physician urgently.
